Summary of FY25 Continuing Resolution

On Sunday, Sept. 22, Congressional leaders released the text of the Continuing Appropriations and Extensions Act, 2025, also known as a continuing resolution, to fund the federal government through Dec. 20, 2025. The legislation is expected to be approved by Congress and subsequently signed into law by the President before Sept. 30, avoiding a government shutdown. The bill provides $27.1 million to address shortfalls in Medicaid funding for the Commonwealth of the Northern Mariana Islands (CNMI) and extends funding for critical autism-related programs.

Outlook

The ASTHO government affairs team is optimistic that the bill will be signed into law, and a government shutdown is highly unlikely. We expect negotiations to potentially finalize full-year appropriations for FY25 to not begin until after the conclusion of the November elections.

Public Health Provisions

In addition to extending federal funding through Dec. 20, 2024, the bill includes the following provisions relevant to state and territorial public health:

  • Provides $27.1 million in Medicaid funding for CNMI from Oct. 1, 2022, to Sept. 30, 2024, to address previous shortfalls in funding. The increase in funding applies to services provided in fiscal years 2023 and 2024, even if payments for services are made in fiscal year 2025. Furthermore, these additional funds will not affect calculations of Medicaid funding for CNMI in later years.
  • Extends funding for the following programs through Dec. 20, 2024:
    • Developmental Disabilities Surveillance and Research Program
    • Autism Education, Early Detection, and Intervention Program
    • Interagency Autism Coordinating Committee
  • As a reminder, funding for the following programs is extended through Dec. 31, 2024:
    • Community Health Centers
    • National Health Service Corps
    • Teaching Health Centers that Operate graduate medical education programs
    • Special Diabetes Program
    • Authority for states and tribes to request temporary reassignment for federally funded personnel
  • Extends the continuation of programs under the Compacts of Free Association with the Republic of Palau between Oct. 1 and signing a new federal programs and services agreement.
