Million Hearts Case Studies

ASTHO is collecting stories about ways states are integrating public health and healthcare to meet the goal of the Million Hearts initiative. States have also shared some resources and materials from their work supporting Million Hearts.

New York Cardiac Population Health Initiative

The New York Cardiac Population Health Initiative (NY CPHI) is a collaboration between the New York State Department of Health and New York’s designated quality improvement organization. CPHI is designed to achieve system-wide practice change and healthcare improvement by providing technical assistance and practice support to primary care practices statewide to improve performance and health outcomes around Million Hearts metrics, including the ABCS of heart health. Ohio’s "Check It, Change It, Control It" Toolkit

The "Check It, Change it, Control It: Your Heart Depends on It" toolkit was produced through a collaboration between the Ohio Department of Health (ODH) and the Ohio Academy of Family Physicians (OAFP). It was designed to increase screening among African-American males for hypertension and high cholesterol and improve their preventive behaviors and treatment by focusing on both patient education and physician training in care management and cultural sensitivity.   • Outcomes Report: The “Check It. Change It. Control It. Your Heart Depends on It.” toolkit pilot project concluded in June 2013, and the final outcomes report—released after the publication of the case study—indicates the toolkit was well-received by both patients and providers. Most participating practices found the toolkit helpful in facilitating high cholesterol and hypertension with their patients. Pre- and post-survey results indicate the toolkit increased the number of patients who knew what healthy blood pressure numbers are by 179 percent, what healthy cholesterol levels are by 132%, and what their own blood pressure numbers are by 23 percent. Many patients also made health behavior changes as a result of discussions with their provider facilitated by the toolkit.

Maryland's Million Hearts Initiative

The Maryland Million Hearts Initiative is a broad state-level effort led by the Maryland Department of Health and Mental Hygiene (DHMH) that leverages existing initiatives and engages many partners across sectors to improve cardiovascular health in the state. Read full case study »

  •  Webinar Recording: MDHMH Works with Partners to Support Million Hearts: This webinar is the first in a series of Millions Hearts webinars being hosted by ASTHO with sponsorship from CDCs Division for Heart Disease and Stroke Prevention. The webinar aims to highlight how Maryland is working to support Million Hearts, discusses innovative partnerships that can help state and territorial health agencies support Million Hearts, and catalyzes the momentum of ASTHO’s Presidential Challenge to Advance the Reintegration of Public Health and Healthcare to support Million Hearts.

Healthy Hearts for Marylanders was a collaboration between the Maryland Department of Health and Mental Hygiene’s Center for Chronic Disease Prevention and Control and the Mid-Atlantic Association of Community Health Centers from 2009 to 2012. The project provided technical assistance to federally qualified health centers across the state to achieve improvements in quality of care and health outcomes—particularly outcomes related to the ABCS of heart health—among patients diagnosed with diabetes. Healthy Hearts for Marylanders Resources: 

Maryland's Patients, Pharmacists Partnerships (P3) is a partnership between the Maryland Department of Health and Mental Hygiene and the University of Maryland School of Pharmacy to improve hypertension and diabetes prevention and control by partnering with community pharmacists to provide medication therapy management services to employees of self-insured employers across the mid-Atlantic region, including Maryland. Iowa's Million Hearts Initiative

Iowa’s Million Hearts Initiative is led by the Iowa Department of Public Health (IDPH) and leverages partnerships across sectors to support components of Million Hearts, particularly improving quality of care around the ABCS of heart health. Key components of this work include supporting team-based care models for blood pressure management, incorporating Million Hearts goals into state-level strategic planning efforts, and educating healthcare professionals and the public about Million Hearts and how to get involved.
