Population Health Forum
The Population Health Forum (formerly known as the Integration Forum and the ASTHO-Supported Primary Care and Public Health Collaborative) is a partnership of organizations seeking to inform, align, and support the implementation of integrated efforts that improve population health and lower healthcare costs. The Forum serves as a platform for primary care, public health, and others to share ideas, learn about each other’s successes and challenges, and create closer connections.
