This report describes the linkages between clinical practices and community organizations and provides recommendations to the Agency for Healthcare Research and Quality on enhancing research efforts, sharing promising models, and promoting policy change to improve preventive service delivery through clinical and community linkages. Clinical-Community Relationships Measures Atlas
An Atlas of existing measures used in research on clinical-community relationships to improve the delivery of clinical preventive services. This Atlas also includes a proposed conceptual framework (model) for measurement of clinical-community relationships (which incorporate partnerships between primary care practices and public health entities).
Core Metrics for Better Care, Lower Costs, and Better Health
A workshop put on by the Institute of Medicine that aims to identify issues, options and approaches to promoting and aligning existing national, regional, and institutional measurement systems to assess progress toward achieving better care for individuals, lower per capita costs, and better health for populations as the natural outcomes of a continuously learning and improving health system.
