Primary Care and Public Health Strategic Map

The Primary Care and Public Health Strategic Map was created in July 2012 with the support of IOM, United Health Foundation, CDC and HRSA. This webpage contains links to a collection of resources that support the implementation of the Strategic Map.

The red oval at the top of the strategic map represents the CENTRAL CHALLENGE that we face in integrating primary care and public health integration over the next two years. The dark blue rectangles, called STRATEGIC PRIORITIES, define the few critical things we need to do to meet our central challenge. The boxes under each strategic priority are called STRATEGIC OBJECTIVES. The objectives spell out more specifically what needs to be accomplished in order to achieve each strategic priority. Clicking on each STRATEGIC OBJECTIVE labeled A1 – E5 will direct you to a separate webpage with a list of resources supporting that objective. 

The boxes highlighted in purple, green, orange, and aqua are the four Tracks of Work, and have been identified as a priority for the next 6 – 12 months by mapping session participants.
