Community Health Teams Issue Report
This issue report demonstrates how collaborations between primary care providers and community health teams can provide improvements in public health and disease prevention in accordance with provisions from the Patient Protection and Affordable Care Act. Additionally, it demonstrates health reform implementation by presenting innovative strategies undertaken by states to link primary care practices with community health teams. Methods of analysis include highlighting the successes and challenges the states Vermont, North Carolina, and Montana have had in implementing innovative programs featuring community care teams.
