Capturing Primary Care and Public Health Integration Successes
This tool has been created to provide both an easy-to-use and comprehensive mechanism to capture examples of integration from both the primary care and public health perspectives. The example(s) you provide will be reviewed and analyzed for key elements and themes to inform ongoing efforts in integration and overall health systems improvement. Please take a minute to think about your own initiatives in government, non-profits, businesses, community organizations and any number of settings, and join us in this effort by sharing your examples of primary care and public health integration.
