Primary Care and Public Health Integration Strategic Map
The first strategic map was
created in July 2012 by key public health and primary care leaders in response to the
Institute of Medicine (IOM) Report titled Primary Care and Public Health: Exploring Integration to Improve
. The map intends to guide the work of ASTHO, our
partners, and others interested in supporting the integration of public health
