Population Health for the Clinician Podcast Series (New Hampshire) 
The NH Medical Society and the UHN Institute for Health Policy and Practice released this 8-part podcast series to provide the clinician with an overview of the principles and practices of population health and provides no-cost CME credit. Episode 5 entitled “Putting Prevention into Action in Your Clinic and Community” highlights the Million Hearts initiative and features successful strategies to incorporate into your practice and work with your community.
