Million Hearts is a national initiative to prevent 1 million heart attacks and strokes by 2017. Launched in September 2011, the initiative brings together public and private partners to enhance cardiovascular disease prevention activities and improve care by encouraging a target focus on the "ABCS":
- Aspirin for people at risk
- Blood pressure control
- Cholesterol management
- Smoking cessation
Public health agencies are a key partner in preventing heart attacks and strokes. They play a role in the identification of best and promising public health interventions for prevention and early identification of heart disease, and have unique opportunities to link with clinical care. As a Million Hearts Partner, ASTHO is supporting state health agencies and other partners in their state to address components of Million Hearts, particularly around blood pressure control.

State Learning Collaborative to Improve Blood Pressure Control
ASTHO, with CDC's support, is working with state health agencies from 15 states, Palau, and the District of Columbia to achieve Million Hearts' goal through a learning collaborative focused on integrating public health and healthcare efforts to improve hypertension. The state teams are utilizing a quality improvement process to partner across sectors to implement best practices and evidence-based policies to identify, control, and improve blood pressure. Million Hearts State Engagement Guide
ASTHO, in collaboration with the American Heart Association, the National Forum for Heart Disease and Stroke Prevention, the National Association of City County Health Officials and the National Association of Chronic Disease Directors, has developed a State Engagement Guide
to assist state leadership with opportunities and actions to engage in Million Hearts. This resource features case studies, tips for holding a stakeholder's workshop, key recommendations for states, and technical assistance contacts.
Key Recommendations: How State Health Agencies Can Support Million Hearts
On May 15, 2013, ASTHO convened a Million Hearts Advisory Roundtable to find innovative strategies for public health and primary care integration to support the Million Hearts initiative. Key recommendations
from this action-oriented meeting outline possible roles for how state health agencies can support the goals of Million Hearts. The recommendations were developed with input from state health officials, ASTHO affiliates, and other national experts, as well as from case studies of initiatives in New York and Ohio that support Million Hearts.
