Nominate an Outstanding Public Health Program for ASTHO's 2017 Vision Awards

The Vision Awards recognize outstanding state and territorial health department programs and initiatives that use a creative approach to address public health needs or problems. Application deadline is Friday, June 9.

Submissions will be separated, evaluated, and recognized in two different categories: Category A (programs with budgets equal to over $250,000) or Category B (programs with budgets under $250,000). All states are limited to two submissions in each Category.

The winners in each Category receive recognition on ASTHO's website and newsletters, and special recognition at the ASTHO Annual Meeting and 75th Anniversary Celebration in Washington, DC scheduled for Sept. 19-21.

Eligibility and Requirements

To be eligible to apply for a Vision Award, a program or initiative must be:

  • Administered by a state or territorial health agency, although it may be in concert with other state, territorial, or local entities.
  • Already implemented or operational for at least one year, but be in existence or development for less than three years.

All submissions must follow the application guidance listed in the next section. Application deadline is June 9. Applications that do not comply with all requirements will not be considered.
